SOCO Clubhouse & SOCO Skateland General Permission Slip & Liability Waiver
Programs Covered: This permission slip applies to all programs and activities offered by SOCO Clubhouse and SOCO Skateland LLC, including but not limited to: hiking, lake time, cooking classes, art programs, field trips, skate sessions, Skatebox Derby, Pugz Not Drugs, UpRISE, LifeSkillz, Brick City Badgers, and any other special events or activities.
Waiver of Liability and Assumption of Risk: I understand that participation in the above programs may involve physical activity, transportation, and outdoor experiences. I certify that my child is in good health and able to safely participate in all activities.
I acknowledge and understand that participation in any program carries a risk of injury, and I voluntarily assume all such risks, including but not limited to illness, accidents, or injuries resulting from participation, transportation, or use of facilities.
Medical Consent: In the event of an emergency, I authorize the staff of SOCO Clubhouse and SOCO Skateland LLC to seek necessary medical treatment for my child. I understand every effort will be made to contact me or the emergency contact prior to treatment.
Program Participation Terms & Conditions: A meal or snack will be provided during most SOCO Clubhouse and SOCO Skateland LLC programs. Please notify us in advance of any food allergies or dietary restrictions.
Registration and Payment Requirements: Participants must be registered and payment submitted at least 24 hours in advance of any scheduled activity or program. This includes Skate Guards and drop-in participants. Failure to register in time may result in the participant being unable to attend.
Cancellations: If a participant is unable to attend a scheduled activity, notice of cancellation must be given at least 24 hours in advance. Failure to notify staff may result in forfeiture of payment and may impact future participation eligibility.
Expectations:
Participants must arrive on time and be picked up promptly at the end of the program.
Repeated no-shows, late cancellations, or behavioral issues may result in removal from future programs.
Families are responsible for transportation unless otherwise specified.
All participants must adhere to program rules and staff directions for safety and enjoyment of all.
By signing below, I agree to release and hold harmless SOCO Clubhouse, SOCO Skateland, its staff, volunteers, affiliates, and partners from any and all liability, claims, or demands arising from my child’s participation in programs.
